Pike Market Case Manager

DEFINITION:
The Case Manager assists patients of Pike Market, who are HIV+ or who have AIDS, to access and to and maintain medical care and treatment adherence while providing referrals and follow up to supportive services including psychological, dental, chemical dependency. This position is the primary case manager for Pike Market and will include case managing clients not at Pike Market as well as assisting with Information and Referral services. This position conducts comprehensive assessments, develops and updates service plans, facilitates the acquisition of all appropriate services, provides financial advocacy and brief advocacy services as needed. This Case Manager reports to the Case Management Supervisor. This is a full-time, exempt, union, regular position that includes working off-site.

CORE RESPONSIBILITIES:
1. Accept direct referrals from Pike Market medical staff.
2. Perform intakes, comprehensive assessments and develop service plans as specified by Title XIX Case Management Standards.
3. Assist clients on a drop-in basis as well as during regularly scheduled appointments.
4. Link people to and serve as a liaison between clients and public and private services by providing referrals or assistance to clients in order to obtain other social services including; home care, mental health services, chemical dependency services, medical services and financial entitlements.
5. Be responsible for client tracking, reporting and related documentation.
6. Collaborate with the medical staff at Pike Market to ensure continuity of care for patients.
7. Work with discharge planners in hospitals to arrange for the services a client needs outside of the hospital setting.
8. Advocate at the program and community level for services for people living with HIV.
9. Attend appropriate trainings to satisfy contractual obligations and case management standards.
10. Be knowledgeable about case management standards and contractual requirements for case management, maintain adherence to these standards and requirements.
11. Attend case management case consultation groups.
12. Perform all assigned duties of position.

ORGANIZATIONAL CORE RESPONSIBILITIES:

5. Demonstrate ability to work effectively and productively with others, including supervisors, colleagues and clients.
6. Regular and reliable attendance, with regular and reliable hours as defined by your supervisor.
7. Knowledgeable about the progression and treatment of HIV/AIDS.
8. Demonstrate ability to work with people of diverse socio-economic backgrounds, ethnicity’s, sexual orientation and lifestyle.
9. Maintain confidentiality of financial, donor and client records.

QUALIFICATIONS:
• Bachelor’s degree or equivalent experience in a social service field
• Five or more years of relevant experience.
• Two years of experience must be related to advocacy services, case management or similar work with the public in a direct service setting.
• Experience working with clients with psychiatric disabilities, chemical dependency and/or other life challenging diseases desired.
• Able to organize, prioritize multiple projects and meet deadlines in a time-sensitive environment.
• Possess an understanding of professional boundaries.
• Demonstrated skills and strong working knowledge of Microsoft Office suite of applications, including Excel, Access and Word.
• Experience working with Provide or similar database is a plus.
• Ability to communicate clearly and professionally in both verbal and written form.
• Proven ability to work independently and in a collaborative team environment. Comfortable working as team member within a medical setting.
• Experience with group facilitation, conflict resolution and a working knowledge of organizational development issues.
• Ability to respond quickly to change.
• Ability to develop good working relationships with internal and external customers.
• Demonstrated ability to work effectively under stressful conditions.
• Access to regular, dependable and timely transportation.
• Ability to occasionally work a flexible schedule, working with clients on a drop-in basis, and handling a large volume of requests for service.
